III. PREFERRED QUALIFICATIONS
- Possess a verified and clear professional degree or qualification in pharmacy at the minimum level of a bachelor’s degree in pharmacy (or higher, such as a Doctor of Pharmacy – Pharm.D.) from a recognized educational institution.
- Licensed or registered as a Pharmacist in your state, demonstrating legal authorization to practice pharmacy with advanced clinical responsibilities.
- Completion of formal training, postgraduate residency (often 1–2 years in clinical settings), fellowship, or supervised clinical experience in pharmacotherapy or clinical pharmacy practice.
- Relevant professional certifications in pharmacotherapy or related specialty areas are preferred, such as Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Critical Care Pharmacist (BCCCP), Board Certified Oncology Pharmacist (BCOP), Board Certified Ambulatory Care Pharmacist (BCACP), or equivalent credentials in advanced pharmacotherapy practice.
- Previous experience providing pharmacotherapy specialist services, including comprehensive medication management, optimization of complex drug regimens, therapeutic drug monitoring, management of high-risk or chronic conditions (e.g., cardiology, infectious diseases, oncology, critical care), prevention and resolution of adverse drug events, interdisciplinary team collaboration, patient counseling, and evidence-based interventions to improve medication safety, efficacy, and therapeutic outcomes across diverse patient populations (in-person or via telehealth where permitted by state law).
